Liability Waiver for Club Diving
In order to participate in any diving event with Big Apple Divers, every diver must have a signed liability waiver and an active dive insurance policy for the current year on file with the Membership Secretary. Before signing this waiver please note:
1. You will be asked to provide an image of your active dive insurance policy in the digital formwork with name, policy number, and date of expiration clearly displayed. Please save a file or screenshot on a device of your choosing before attempting the formwork.
2. After filling out the digital formwork, you will also be asked to verify your signing via an email from Smartwaiver. Your waiver is not complete until you complete this verification.
If you would like to submit an updated depth certification or have any questions, they may be submitted to membership@bigappledivers.com . The only submitted certifications should be: OW, AOW, DEEP, AN/DP, Norm. Trimix, or Hypoxic Trimix.
Note: Smartwaiver provides secure, cloud-based storage for legally binding digital waivers, compliant with laws like ESIGN and GDPR, ensuring data safety through encryption and security certifications.
