In order to participate in any diving event with Big Apple Divers, every diver must have a digital copy of the club's liability waiver for the current year on file with the Membership Secretary. We do not presently have the capability to work with digitally signed legal documents.

The waiver MUST be hand initialed on the first and second pages, and the third page must be hand signed and fully completed, INCLUDING the witness section. Dive insurance is also mandatory for all divers joining our club dives, so this section must be completed as well, documenting insurance whiich will be in force at the time of any club dives.
